Physical and Concussion Forms Required to Participate!

The Kansas State High School Athletic Association (KSHSAA) requires that all students who will be competing in athletic activities have a current physical form and a current concussion form filled out and signed.  These forms must be signed and on file at the school before the athlete can practice. The following is taken from the KSHSAA handbook.  

Rule 7 PHYSICAL EXAMINATION PARENTAL CONSENT

Section l: General Regulations (apply to grades 7-12) Art. 1: No student is eligible to represent any school in interschool athletics, spirit or spirit competition groups until there is on file with the superintendent or principal, a statement certifying that the student has passed an adequate physical examination and is physically fit to participate in interschool athletics, spirit or spirit competition groups.

These forms can be downloaded and printed from www.kshsaa.org, or you can pick them up from school.  There will be copies available at enrollment.
